Your First Appointment
What to Expect
When you come for your first appointment, you will be asked to fill out forms that provide background information about your health. We ask you come 10-15 minutes early to fill out this paperwork. If you would like to save time, you can download the New Patient Intake Forms below and take them to your first appointment. Following, you will have a one-on-one consultation with the Doctor. The Doctor will perform a thorough orthopedic, neurological and chiropractic exam. X-rays may be taken but not always. We encourage you to bring any recent films to this visit.
Once a diagnosis is established, the Doctor will determine if you would benefit from chiropractic care. There will be a treatment plan individualized to your condition which will cover the type of treatment, the length of treatment and recommended home care. Short term goals may include reduction of pain and restoring motion. Long term may include functional independence and wellness care.
Please note we do not sell extended treatment plans or require pre-payment packages. Our doctors understand each patient's condition is different from another patient. Your course of treatment will depend on your condition, severity and your health goals.
